Temple Beth Shalom COVID-19 and Flu Clinic

Holtzman Medical Group, in partnership with Temple Beth Shalom, will be operating a pediatric COVID-19 and Flu vaccination clinic on Thursday, December 5th 2024, from 3:00pm-6:00pm. This clinic is exclusively for children ages 6months - 18 years.

Please read all information below before booking.

COVID-19 vaccines:

Holtzman Medical will be carrying the the updated 2024-2025 Moderna vaccine (monovalent, KP.2). The COVID-19 vaccine will be available at no out of pocket cost. We will bill your insurance for both the vaccine and its administration. However, adults without insurance, or those whose insurance does not cover the vaccine (including some cases where HMOs deny claims due to out-of-network issues), may need to seek alternative options. Unfortunately, the Bridge to Access program (funding the gap last year) ended in August due to lack of federal funding. Individuals may choose to pay out of pocket at our clinic, with the following pricing: $175 COVID-19 and $25 FLU.

Staying Up to Date with COVID-19 Vaccines:

  • Unvaccinated children 6 months through 4 years of age are eligible to receive 2 doses of the updated 2024-2025 Moderna COVID-19 vaccine

  • Children 6 months through 4 years of age who have previously been vaccinated against COVID-19 are eligible to receive one doses of an updated 2024-2025 mRNA COVID-19 vaccine (timing and number of doses to administer depends on the previous COVID-19 vaccine received)

  • Individuals 5 years of age and older regardless of previous vaccination are eligible to receive a single dose of an updated 2024-2025 mRNA COVID-19 vaccine

  • Individuals ages 12 years and older regardless of previous vaccination are eligible to receive a single dose of an updated 2024-2025 mRNA COVID-19 vaccine

  • Individuals 65 years and older an additional COVID-19 vaccine dose— beyond what is listed above—is NOT currently recommended unless you are moderately or severely immunocompromised

People who are moderately or severely immunocompromised may receive 1 or more additional updated COVID-19 vaccine doses. Please speak with your primary care provider to determine what is right for you.

FLU VACCINES:

Holtzman Medical will be carrying the Flulaval quadrivalent vaccine - standard strength.

PAYMENT AND INSURANCE INFORMATION FOR FLU:

  • If you have a PPO or other health plan that does not require a referral you can receive the flu vaccine at no out-of-pocket cost at our clinic.

  • If you have an HMO or health plan which requires a referral, or do not provide insurance information you must pay $25 at check-in to receive a Flu vaccine. 

Everyone ages 6 months and older is recommended to receive a 2024-2025 flu vaccine and can receive one at our clinic.

Please include both your insurance provider and policy number in the “insurance” section.

What do I need for my appointment?

Plan to arrive on time, and no more than 10 minutes before your scheduled appointment time.

All children under 18 must have signed parental consent forms for each vaccine. These will also be available onsite.
