The State of Massachusetts has implemented a digital COVID-19 vaccine card at https://myvaxrecords.mass.gov/ This website utilizes the state’s immunization registry to create your vaccine card. Holtzman Medical automatically sends all immunizations you receive with us to the state immunization registry.
